JOB LOCATION: MERCY HOME CARE, 1236 Main Street, HOLYOKE, MA 01040 Provide Physical Therapy services in the home based upon the Plan Of Care as developed by MD and therapist. Provide visits to physical therapy assistant as needed under the POC and oversee the physical therapy assistant care. Must have Massachusetts PT license, as well as valid drivers license.
