Position Description:

Location: Cooley Dickinson Health Care Corporation, Northampton, MA. Req #: VNA12037 Department: VNA/Hospice Schedule: Per Diem - Day Shift - Hours: 8am-4:30pm Main Function: Under the direction of the Director of Business Development builds, establishes, and maintains strong referral relationships with physicians and physician practices, skilled nursing and assisted living facilities, hospital and discharge case managers, and the community ensuring all patients who would require and benefit from VNA & Hospice services have access to those services. Promotes and educates about VNAH programs and services. MINIMUM REQUIREMENTS: · Education: graduate of an accredited school of nursing, physical or occupational therapy; Bachelor’s degree or equivalent preferred. · Experience: 2 years experience in business, sales and/or marketing preferred. · Experience preparing and providing education programs preferred. · Three to five years experience working in Hospice and Homecare preferred · Certification/Licensure: Current Licensure in Massachusetts as a professional nurse, PT or OT required. Valid driver’s license. · Demonstrated excellent interpersonal, organizational and communication skills required.
