PYRAMID is a community-based rehab
program for adults with physical handicaps and developmental delays. Focus is on skill-building, communication skills and community integration.)
Essential Job Functions
• On referral, evaluate member to determine the abilities and need areas in cooperation with the physician and previous reports. Document all findings.
• Develop a treatment plan outlining goals and areas to be addressed.
• Administers treatment utilizing appropriate speech therapy techniques.
• Instructs assigned program staff to provide carryover treatment in accordance with assessment and goals.
• Responsible for ensuring each assigned members occupational therapy objectives are being addressed.
• Gather data and document each member’s progress in meeting objectives and present to Rehabilitation Coordinator prior to member ISP meeting or present at required meetings. Actively participate on member’s interdisciplinary team.
• Communicate frequently with Rehabilitation Coordinator on incidents and successes of each member.
• To assistant the Program Manager in the screening of prospective participants, as assigned.
• To foster the growth of individuals by teaching and offering opportunities for choice, initiation and risk taking.
• To treat individuals with respect and dignity and in an age and culturally appropriate manner.
• To act as a role model and provide contextual training of staff in the integrated therapeutic model.
• Maintain all Occupational therapy documents and records in accordance with DPH regulations and standards; Assessments, Care Plans, Goals sheets, Progress Notes and Discharge summaries.
• Study current trends and new developments in occupational therapy offering new ideas and practices.
• Perform others duties as assigned.
Qualifications
1. Possess current Occupational Therapist license.
2. Strong communication, interpersonal and listening skills.
3. Respect for the rights and abilities of people with disabilities.
4. Valid driver’s license and good driving record.
5. At least 21years old.
